Program Name: Pro Care Innovations Patient Care Communications

Program Description: Pro Care Innovations ("PCI") offers SMS text messaging to patients enrolled in our Chronic Care Management (CCM), Principal Care Management (PCM), and Remote Patient Monitoring (RPM) programs. These messages may include care coordination follow-ups, appointment and check-in reminders, and general communication from your care team.

Consent: By enrolling in a PCI care management program, you consent to receive SMS text messages from Pro Care Innovations related to your care. Consent is obtained verbally during your enrollment call and documented in your patient record. You are not required to consent to receive text messages as a condition of receiving care services.

​

Message Frequency: Message frequency varies depending on your care plan and program enrollment. You may receive up to several messages per month.

​

Message and Data Rates: Message and data rates may apply based on your mobile carrier plan. Pro Care Innovations does not charge for sending or receiving these messages, but your carrier's standard messaging rates may apply.

​

Opt-Out Instructions: You may opt out of receiving text messages at any time by replying STOP to any message. After opting out, you will receive one final confirmation message and no further texts will be sent. Opting out of text messages does not affect your enrollment in, or access to, PCI's care management programs.
