At Pro Care Innovations, our Care Team Coordinators partner with patients living with chronic conditions to help them better manage their health, stay connected with their providers, and reduce unnecessary hospital visits.

What You'll Do

Provide monthly care coordination and support to Medicare patients by phone.

Build meaningful relationships with patients through compassionate communication.

Develop and update individualized care plans.

Educate patients on managing chronic health conditions.

Coordinate care with physicians, specialists, hospitals, and other healthcare providers.

Accurately document patient interactions in our electronic health record (EHR) system.
What We're Looking For

Current RN, LPN, or Medical Assistant (MA) credentials.

Strong computer skills with the ability to navigate multiple applications while speaking with patients.

Excellent verbal and written communication skills.

Strong organizational, critical thinking, and documentation skills.

A compassionate, patient-focused approach to care.

The ability to work independently while collaborating with a supportive clinical team.
