NOW HIRING CARE TEAM COORDINATORS

Do you have experience in health care? You can make a difference outside of direct patient care. Due to tremendous growth, we are hiring Care Team Coordinators in all three of our locations. As a member of our chronic care team, you will have the opportunity to improve the quality of life for our patients by delivering one-on-one clinical guidance and support.

​

Together, we're removing the barriers that prevent patients from receiving timely health care in hopes of keeping them out of the emergency department. We focus on collaborating with primary care physicians, specialists, and hospitals to create a comprehensive, individualized care plan around the patient's chronic conditions.

​

Your primary responsibility will be to provide at least 20 minutes of care coordination for Medicare patients each month. Key tasks will include telephone interactions with patients, documentation of patient interactions, care plan development, chart reviews, care coordination between providers and other facilities, and the ability to work well with the patients and providers to optimize patient care.

Job Description:

We are seeking dedicated Care Coordinators to assist us with patients enrolled at our facility. You will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient-care services, educating them about their condition, and working with the care team to evaluate interventions.

​

Position requires:

  • Strong computer literacy (Microsoft Windows, keyboarding skills, strong systems aptitude, etc.)

  • Strong computer & telephone multitasking skills including: the ability to effectively search for and type information on the computer, navigate through multiple windows and screens quickly, and inputting information accurately while keeping pace with the call.

  • Excellent verbal and written communication skills

  • Exceptional people skills with computer knowledge and the willingness to learn

  • Must be a self-starter with excellent communication skills and patient assessment skills.

  • Excellent interpersonal skills with patients, Primary Care Physicians, Specialists, and colleagues are a must.

Regular office hours are 8:00 a.m. to 5:00 p.m. Monday to Friday.

Job Type: Full-time

Pay: $14.00 - $20.00 per hour
