What a fantastic day for Pro Care Innovations. We started providing Chronic Care Management in May of 2017 with a Vision of Shifting the outcome of our geriatric population through Preventive Care. Pro Care is excited to announce that as of today we have completed of 250k CCM's and are actively pursuing our vision!
We have continued to partner with great clinics throughout 2020 and are excited to continue serving our patients and clients through preventive medicine. In 2021 we are looking to continue the expansion of geriatric patients receiving preventive medicine through Chronic Care Management. We are also very excited to begin offering Remote Patient Monitoring to our patients as another avenue for preventive medicine.
Pro Care Innovations currently serves the following specialties : Urology, Oncology, Orthopedics, Primary Care, Cardiology, Pain Management, Endocrinology, and more. We have clients in Texas, Arkansas, Missouri, Oklahoma, Missouri, Kansas, and more. We currently serve over 15,000 patients a month and growing.
We wanted to give a shout out to our amazing team of Care Team Coordinators ( CTC's) who call our patients everyday to provide the highest levels of preventive medicine. Our CTC's pursuit to see our vision come to pass in unmatched.
