Pro Care Innovations is pleased to announce their partnership with Access Care Oklahoma. Access Care Oklahoma is an Accountable Care Organization (ACO) with more than 400 providers in Oklahoma. ACOs are groups of doctors, hospitals, and other health care providers, who come together voluntarily to deliver coordinated high-quality care to their Medicare patients.
Two ways to deliver this care include the Annual Wellness Visit (AWV) and Chronic Care Management (CCM).
The AWV is a yearly appointment to discuss a plan of preventive care in the coming year. It is a covered benefit for all patients who have been on Medicare Part B for twelve months. It is designed to lower health care spending costs by encouraging patients to have preventive screenings performed in a timely manner.
CCM services are specifically for Medicare patients who have two or more serious conditions that are expected to last at least a year. This includes conditions like arthritis, asthma, diabetes, hypertension, Obesity and more. They receive monthly check-in calls and other services to ensure their condition is being managed between in-office appointments.
Pro Care Innovations will be working with Access Care Oklahoma to help provide both of these services for patients.
